If you are not completely satisfied with

ProActive Nutra will will refund 100% of the suggested retail price
and any shipping charges if applicable.


To receive your refund, send ALL of the following items:

1.  The UPC Code. – The entire box or bottle will also work.

2.  Original sales receipt – showing product purchased, date of purchase, and name and address of retailer. If you purchased via the ProActive Nutra web site; please include a copy of the invoice shipped with your order.

3.  A letter detailing your dissatisfaction with the product.

4.  Photocopy of your driver’s license or other picture ID.

5.  Phone number with area code where you can be reached during office hours.  – A representative may contact you to discuss your results and/or make suggestions for future success.
